Terms and Conditions - October 2007
1 Registration Conditions
1.1 Personal details must be confirmed by one of the following:
• Evidence from your employer or voluntary organisation
• An existing registered user
• A member of Health Promotion staff1.2 Both work and home contact details must be supplied if you wish to borrow items. Home details are not needed if only leaflets and posters are required.
1.3 Users will keep the Resource Service updated with any changes to their details.
1.4 Users will inform Resources of any problems with our items, such as being out of date, having incorrect contents or better items being available. Professional feedback is a vital part of our service.
1.5 All registered users must abide by these terms and conditions.
2 Borrowing Items
2.1 Clients may only borrow medical equipment (e.g. blood pressure monitors, resuscitation models, etc.) if they have had formal training and know properly how to operate them.
2.2 Registered users may have up to six items on hire at any one time. Bookings may be made for periods of up to two weeks and extended for further two week periods if still available. If you require more items at one time, this may be possible. Just give us a call.
2.3 Bookings made less than four working days in advance are not guaranteed to arrive on time.
2.4 Hire periods may be extended up to a further two weeks during the final four days of a hire period.
2.5 Any items on hire to you are totally your responsibility for the period of the hire until they arrive back at the Resource Service.
2.6 Replacement or repair of any items lost or damaged while your responsibility will be charged to you / your employer.
Each case will be treated individually.2.7 If a user passes an item on to another person without amending the booking with us (confirmed by the new borrower) then the item remains that user's responsibility.
2.8 The return date on the booking advice slip is the date the item/s must be put back in the post to the Resource Service.
2.9 All items will be returned on time.
Remember - Late returns wreck the careful planning of other users! and it might happen to you next.
The most common cause of items not being available on time is late return.2.10 Users have two days grace for late returns before we contact them for the item's return.
2.11 Any users with items currently overdue for return may not order or borrow any further items.
2.12 Once an item is one month overdue for return we will consider it lost and charge the user / their employer for its replacement.
3 The Resource Service will
3.1 Maintain a well stocked resource library as far as is realistically possible, within the limits imposed by our suppliers and demand from our users.
3.2 Maintain the quality and accuracy of material as high as possible through research and user feed-back. However, due to the vast range of items and subjects covered, we do not accept any liability for the content of items. It is the user's responsibility to review items properly before use to ensure they are suitable for their particular needs.
3.3 Ensure the quality of the items in the library by checking every item as they are sent out and returned.
3.4 Endeavour to meet any reasonable request from a user.
3.5 Consider every constructive comment from our users .
3.6 Be prepared to be flexible with these rules when appropriate in individual circumstances.
